EdChoice Scholarship from State of Ohio
Enrollment at St. Gabriel School must be secured prior to applying for EdChoice.
STEP ONE: Complete following forms and return to Michelle.Clarke@St-GabrielSchool.org or bring to office.
The Ohio Department of Education has implemented a new Scholarship Income Verification System. The system is available for families to enter and submit their income information electronically to the Scholarship. Parents are able to enter household member information, income information and upload supporting income documents directly into the online system.
- EdChoice Request Form (Complete one for each child) OR EdChoice Renewal Form (must have signature)
- SUBMIT WITH PROOF OF RESIDENCY (this is a utility bill with home address AND utility company's address)
- SUBMIT WITH COPY OF BIRTH CERTIFICATE
